note 1. MoneyCard Catalog lists 10,000 released in Feb, 1994. My card is dated 1995 and my records indicate 50,000 were released in Aug, 1995.
note 2. MoneyCard Catalog lists two Connections/Hispanic
cards, #USW 62- a Show card (mintage 1,000) and #USW 63- a Collector
card (mintage 250). My records and the actual
card indicate one card produced with a mintage of 1,250. A possible
explanation is that US West released 1,000 cards to the show promotor
and retained 250 cards for distribution. I don't believe two
different cards exist.
note 3. 1010 cards were initially produced. 10 cards were kept by US West and 1,000 cards were issued to Global Telecard Co. 863 were returned because of flaws in the chip specifications, but were eventually available to authorized distributors. To distinguish the flawed cards, holes were punched in them and sold wthout the foldersd (produced and retained by Global Telecard Company). There was speculation that the 863 flawed cards were replaced with corrected cards to honor the commitment and complete the original private order. Therefore, there may be a total of 1,873 cards in existence. US West also learned how the collector's market was manipulated when an entire issue was retained by the private issuer and subsequently insisted on retaining at least 100 cards of every issue to release through its authorized distributors.
note 4. Listed in MoneyCard Catalog, but never produced.
note 5. Out of 1,500 sets produced, (without expiration date!) 500 sets were released without folders, 1,000 sets were released with folders and of these, only 100 were autographed by the artist. Contains the first $15 denomination US West card with the 'Life's Better Here' theme plus 14 cards, each with a drawing depicting the highlight of each state in the US West service area. Most were sold internally to US West employees, so few were available to collectors.
note 6. Five of the US West service area states with 'Changing The Shape Of Money' title, but the same images used in the Life's Better Here series.
note 7. US West began producing Sample cards, in addition to Regular cards, in January, 1997. Mintages vary from 25-50 cards per issue.
note 8. MoneyCard Catalog lists this card as $5 denomination. At 5¢ per minute (same as the other Schlumberger Test cards) the total value would be 100 units. However, the actual card has a printed value of 105 units, equivalent to $5.25.
